All patients attending the department must be referred by a medical or dental practitioner using the standard referral forms available from Bradford Health Authority. Patients should not be advised to attend the department without a referral letter. Internal referrals from other hospital specialists are welcome and encouraged. Referral to a specific named consultant is encouraged. Where no consultant is specified the referral will be allocated to the consultant with the shortest waiting list. Upon receipt, referrals will be allocated a priority by the receiving consultant.
Urgent refrerrals for suspected head and neck cancer should be made using the specific "14-day wait fast track" proforma. Other urgent referrals should be made by telephone or fax directly to the department. These should always be followed by a written letter of referral to be included in the patient's hospital notes.
Orthodontic referral forms are available from the Department on request, or from Bradford Health Authority. Routine referrals should be made using these referral proformas. All referral forms/letters are screened by the Consultant concerned and the priority of the referral (either urgent or routine) decided upon. All patients receive an acknowledgement of their referral and an approximate length of wait. After the patient has attended the Department for their initial appointment, a report is sent back to the referring source with copies sent (as appropriate) to the patient's General Dental Practitioner and General Medical Practitioner.
